docs: explain each cause behind an empty generation output

Changes

Adds a troubleshooting entry, "Why does my generation show no output?", to the AI Observability troubleshooting page.

The AI observability trace view is gaining a notice that names why a generation rendered no output (PostHog/posthog#90159). This page gives each sentence the notice can show a row: what happened, and what to do about it. It answers the review feedback on that PR that the messages leave users asking "okay, so how do I fix this?"

Two of the sentences being documented, as they render in the trace view:

Best merged before PostHog/posthog#90159 lands: that PR links each notice to this section's anchor (#why-does-my-generation-show-no-output), so the heading text is load-bearing.
